Description
The New York State Office of Mental Health seeks a qualified, New York State-licensed optometrist to provide on-site optometry services to consumers at the New York City Children's Center Queens Campus. Services include evaluating consumers and developing treatment plans, reviewing medical histories and treatment goals, performing and interpreting tests, providing indicated treatment, documenting services and progress, and consulting with treatment teams. The provider will work under the Director of Health Services, coordinate with facility staff, and submit monthly service statistics. Service hours will be mutually agreed upon. The facility will provide supplies and materials. The bid quote sheet requests pricing for eye examinations, spectacle fittings, frames and cases, polycarbonate, and single-vision and bifocal lenses. Bidders must submit the completed bid quote sheet and the listed certifications, references, responsibility materials, insurance and licensing evidence, and applicable supplier-diversity forms. The contract term is April 1, 2027, through March 31, 2032.
