Gloucester County Improvement Authority seeks a New Jersey-licensed ophthalmologist to provide recurring eye care services for residents of Shady Lane Home during a one-year contract term. The scope includes monthly scheduled visits, ensuring each resident is seen at least annually, emergency visits as needed, compliance with applicable regulations, notification of the facility regarding changes affecting residents' eye care, and billing Medicare, Medicaid, and private insurance. The ophthalmologist must be able to enter orders and documentation in the Sigma Care System, use facility-stock ophthalmic medications when possible, and engrave each resident's name on the rim of dispensed glasses. The cost proposal must identify any proposed retainer, services covered by the retainer, and hourly billing rates; travel expenses will not be reimbursed. A copy of the New Jersey professional license and current malpractice insurance must be submitted.
