The Massachusetts Executive Office of Health and Human Services is accepting applications from eligible rural hospitals, federally qualified health centers, rural health clinics, healthcare providers, municipalities, community-based organizations, nursing facilities, and other qualifying rural entities for participation in the Rural Health Transformation Program. Available support includes technical assistance, planning and implementation assistance, workforce development, population health initiatives, telehealth and digital health activities, cybersecurity, care coordination, direct funding, equipment and technology investments, and facility modernization. Applicants may apply for one or more eligible activities through a common application. The initial participation period is October 30, 2026, through September 30, 2027, within a broader five-year program. Applicants must submit the completed Rural Partner Application and, when applicable, a Partner Validation Form for each named partner. Certain activities identify specific funding limits, including up to $100,000 for each Year One regional MYCare network, up to $200,000 for remote patient monitoring participants, up to $480,000 per specialty care network, up to $250,000 per Rural SHINE-HT project, and capital funding amounts ranging from approximately $125,000 to $2.5 million depending on the activity and facility type.
