Description
The City of Louisville is soliciting bids to mill the existing surfaces and resurface West Muhammad Ali Boulevard and West Chestnut Street. Bidders and disadvantaged business enterprise subcontractors must be prequalified by the Kentucky Transportation Cabinet and hold a Certificate of Eligibility at bid opening; other subcontractors must be prequalified when accepting subcontracts. A bid bond of at least five percent of the total bid is required. The contract has a zero-percent disadvantaged business enterprise goal. Required submission items include the Vendor Information form, Certification Forms, W-9, Bid Bond, and the IFB270050 Bid Table (BT-05CV) completed using the provided response template. A Certificate of Insurance is listed as optional.
