Description
Salt Lake City School District is requesting Statements of Qualifications from vendors that can provide food items and other products for fundraising activities at schools throughout the district. The resulting process will establish an approved vendor list pursuant to Utah Code Chapter 22, the Charitable Solicitations Act. Responses require completion of online certifications and submission materials, including Attachment C limitations and prohibitions, Attachment D fundraising details, an insurance certificate, Utah business licensing, applicable Utah fundraiser authorization or exemption documentation, IRS Form W-9, qualification and experience information, at least three current client references, and a complaints and problem-resolution description. The solicitation also requires vendor information, conflict-of-interest and debarment-related disclosures, and additional compliance certifications. Electronic responses must be submitted through the designated procurement portal; hard-copy, faxed, and emailed responses are not accepted.
