Description
The County of Stanislaus is seeking qualified professionals to schedule, coordinate, and facilitate Child and Family Team (CFT) meetings for families, youth, and non-minor dependents referred by Child Welfare Services and the Probation Department. Services include contacting families and participants, arranging meetings, facilitating discussions, documenting meetings and service plans, using the Child and Adolescent Needs and Strengths assessment to inform case planning, tracking cancellations and meeting timeliness, and supporting transition planning and collaboration with other providers. Meetings generally must occur at least every six months, every three months for certain Katie A. subclass members, and within specified shorter timeframes for initial, expedited, and emergency meetings. The contractor must provide at least two rooms for CFT meetings, with virtual meetings permitted only with prior written approval for applicable placements. Facilitators must have a higher education degree, human-services experience, and strong communication, interpersonal, and conflict-resolution skills. The contractor must hire and retain qualified staff, provide monthly and quarterly reporting, submit outcome-based scorecards, and meet applicable confidentiality, data-security, background-clearance, subcontracting, and insurance requirements. Proposals must include qualification and technical information, government references, financial information, a pricing proposal and budget narrative, and required signed forms. The available funding is $784,252 for the July 1, 2027, through June 30, 2028, period, with possible annual extensions.
