Description
The New York State Office of Mental Health is seeking quotes for full-service pest elimination services at the New York City Children's Center's Queens, Brooklyn, and Bronx campuses. The scope requires interior and exterior integrated pest management, including inspections, detection, identification, and elimination of rodents, cockroaches, ants, spiders, other arthropods, occasional invaders, and wildlife. Group I services must be provided twice weekly, Monday through Friday between 7:00 a.m. and 6:00 p.m.; Group II and special or emergency services are provided as requested. The contractor must furnish all labor, materials, equipment, reporting, emergency response, pest-monitoring records, floor plans, staff training, and safe pesticide applications. Requirements include New York State pesticide business registration, appropriately certified pesticide technicians, an entomologist, an on-site supervisor, technician background-screening compliance, and adherence to chemical-safety and facility-approval procedures. Quotes are evaluated based on lowest cost, price reasonableness, and vendor-selection justification. The solicitation establishes subcontracting goals of 16% MBE, 14% WBE, and 6% SDVOB; it also states a 30% combined MWBE goal elsewhere in the RFQ. Addendum 1 requires the signed addendum to be dated and included with the proposal behind the Attachment C cover sheet.
