Description
The Town of Colchester is seeking a qualified contractor to perform a water and sewer DOT pavement patch at 200 Lebanon Avenue. The work includes milling and paving an approximately 40-foot-by-20-foot area from the shoulder to the centerline, using asphalt patching and pavement-repair methods that meet Connecticut Department of Transportation Form 819 requirements. The contractor must provide all labor, materials, equipment, mobilization, demobilization, safety measures, and traffic control, including a police flagger for all traffic patterns, under the contractor's own DOT road-entry permit. Bidders must submit the enclosed bid sheet, bid guaranty, non-collusion affidavit, statement of bidder's qualifications, and certification regarding equal employment opportunity. The contractor must be licensed in Connecticut; a copy of the license is required before work begins. The solicitation also requires specified commercial general liability, automobile, excess/umbrella, owners and contractors protective, and workers' compensation insurance, with a certificate naming the Town of Colchester as an additional insured due at award. The contract uses a lump-sum price, includes a 5% retainage, and requires monthly payment applications.
