Description
The Utah Department of Health and Human Services is seeking vendors to provide Pre-Admission Screening Resident Review (PASRR) Level II evaluations for individuals seeking admission to, or residing in, Medicare or Medicaid certified Nursing Facilities. These evaluations are for individuals suspected of or diagnosed with SMI, based on referrals from local NFs, hospitals, and other allied agencies or individuals. This is an open-ended Request for Statement of Qualifications (RFSQ) that will result in an approved vendor list (AVL). All solicitation documents are posted on the DHHS Purchasing site.
