The Department of Public Health seeks a provider, or coordinated network of providers, to deliver one selected community-based care model for pregnant San Francisco residents enrolled in or eligible for Medi-Cal. Eligible models are community-based doula support, non-clinical midwifery support, group prenatal care, or an innovative model incorporating doula or midwifery care. Services must include relationship-based support, prenatal education, care navigation, coordination with clinical prenatal providers, and active referrals and follow-up for priority social needs such as housing, food, behavioral health, safety, transportation, and benefits. The provider must serve participants facing elevated risks of adverse maternal and infant health outcomes and coordinate support through birth and the postpartum transition. Proposals must demonstrate minimum qualifications, including at least three years of cumulative experience in doula, midwifery support, or prenatal care navigation services, supported by third-party documentation. The selected provider will develop a 12-month work plan, collect and report performance and equity data, participate in continuous quality improvement, and plan for sustainability. The anticipated contract is a one-year initial term with nine one-year options. Compensation is cost-reimbursable up to a not-to-exceed amount established in the resulting agreement; the RFP cites an estimated annual amount of $175,000 for year one.
