Description
Cook County is soliciting bids for approximately 133 line items of autopsy supplies for its Medical Examiner's Office. The requested products include autopsy and dissecting knives, scissors, blades, needles, syringes, forceps, hemostats, scalpels, surgical and autopsy gloves, gowns, aprons, masks, respirators, goggles, face shields, shoe covers, specimen and toxicology containers, formalin labels, evidence bags and envelopes, swabs, sponges, absorbent materials, disinfectants, and related supplies. Quantities are estimated and products will be ordered as needed; bidders must price every line item through the online bid table. Products must be new, sealed, and undamaged, and items with expiration dates must have at least one year of shelf life from the order date. The contract term is three years with one two-year renewal option. Delivery is required within five business days of an order, with inside delivery to the Medical Examiner facility. The solicitation requires an electronic submission containing the completed online bid table, Economic Disclosure Statement, Identification of Subcontractor/Supplier/Subconsultant Form, and MBE/WBE Utilization Plan, including applicable Letters of Intent or a Petition for Reduction/Waiver. The contract has 10% MBE and 10% WBE subcontracting goals.
