Description
Elkhart County seeks a contractor for Project No. 25003, intersection improvements at County Road 22/Orpha Drive and County Road 37 in Middlebury. The work includes widening the roadways to add left-turn lanes, realigning the Pumpkinvine Nature Trail, constructing sidewalks and approaches, installing roadway pavement and markings, relocating signs, and maintaining traffic with closures and detours as shown in the plans. The project also includes stormwater drainage improvements with open ditches, inlets, pipes, manholes, erosion and sediment controls, revetment riprap, and retention basins; sanitary sewer installation along County Road 22/Orpha Drive and County Road 37; water-valve adjustment and a fire-hydrant assembly; clearing, earthwork, subgrade treatment, aggregate, HMA paving, concrete approaches, detectable warning surfaces, seeding, and sodding. Bidders must be currently pre-qualified by the Indiana Department of Transportation. The contractor must submit the County's proposal forms, an itemized proposal, a Contractor's Financial Statement from Indiana Form 96, proof of INDOT prequalification, a non-collusion affidavit, and a 10% bid bond. A performance and payment bond equal to 100% of the contract price is required after award, and the contract documents require specified insurance coverage.
