Description
Mohawk Medbuy Corporation is conducting a Request for Supplier Qualification to establish a prequalified supplier list for preventive maintenance and on-call HVAC services. Work will be performed on an as-needed basis to meet hospital-specific requirements across three service zones in Central West Ontario. Proponents may select the zones and service categories for which they seek prequalification, and the participating purchaser schedule identifies hospitals by zone. Submissions require qualification forms, reference information, conflict-of-interest documentation, service-category questionnaires, and applicable U.S. vendor declarations. The revised Zone Response Form identifies the zones requested for prequalification. Proponents may also submit a redlined agreement for consideration, although material changes may reduce the total score by five points. No precise service volume is guaranteed.
