Description
SUNY Stony Brook University Hospital intends to purchase SQUID Liquid Embolic Agent and related items manufactured by Balt USA. The procurement is a discretionary purchase under New York State Education Law Section 355(5)(A), with an indicated authority of up to $125,000; no sealed bid will be held. Interested vendors must contact the designated campus contact within 15 business days of the advertisement notice to provide a proposal. The successful contractor must complete and submit Vendor Responsibility documentation, documents related to New York State Finance Law Section 139 and New York State Tax Law Section 5-a, and valid, current New York State Workers’ Compensation and Disability forms. The procurement is subject to New York State lobbying-law restrictions, and the listed contact is the only permissible contact.
