Description
The University of Utah, through the Utah Education and Telehealth Network, is soliciting proposals for a 20-year, two-strand dark fiber indefeasible right-of-use service. The solution must provide dedicated, high-availability, high-bandwidth connectivity between West Valley Hospital and University of Utah network locations, with all fiber and routing equipment dedicated to UETN. Proposed services must support data, voice, video, and the confidentiality, integrity, and availability requirements applicable to electronic protected health information. The scope includes any required fiber construction, right-of-way costs, installation, special construction charges, non-recurring charges, monthly recurring charges, ongoing fiber operations and maintenance, 24/7/365 monitoring and customer support, outage notification and restoration commitments, service-level agreements, technical documentation, network and logical diagrams, fiber-path mapping, fiber characterization, and any required amplification-site space and power for spans exceeding 80 kilometers. Vendors must submit separate cost information, a technical proposal, SLA, financial responsibility evidence or ISO 9001 certification, a standard dark fiber IRU contract, an exceptions/compliance summary, questionnaire responses, references, and the completed Attachment A cost proposal.
