Description
The City of Omaha is seeking a contractor to renovate the parking lot at Levi Carter Park, 3666 Abbott Drive, Omaha, Nebraska. The base work includes removal and replacement or repair of deteriorated asphalt pavement, related demolition and disposal, grading and compaction as required, traffic control, and striping of parking stalls, including ADA stalls. The contractor must furnish all labor, materials, equipment, hauling, disposal, barricades, and incidental work necessary for completion. The bid includes two add alternates: asphalt parking-lot and drive work at the Levi Carter Boat Ramp, 5155 North 12th Street East, and asphalt parking-lot and entry work at Levi Carter Park, 4405 Carter Lake Shore Drive. Bidders must submit the electronic bid response and required attachments, including bid security, a certificate of insurance, the Responsible Contractor Compliance Form, Employee Classification Act affidavit, Certificate of Compliance, SEB Participation Disclosure Form, and an Economic Sustainability Program plan or approved certificate. The Apprenticeship Incentive Form is optional for bidders seeking the stated incentive. A performance bond, payment bond, and warranty bond are required after award, each for 100% of the contract price.
