Description
The selected consultant will assess existing diabetes-care services, patient and provider needs, care-coordination gaps, institutional capacity, and community resources. The work includes stakeholder engagement; evaluation of navigation models and referral pathways; and recommendations for program structure, staffing, workflows, technology, partnerships, and financial sustainability. The consultant will develop a phased implementation roadmap, estimated costs, performance measures, a comprehensive needs assessment and strategic plan, and a final presentation. This is a planning and program-development engagement; implementation or operation of the navigation program is not included unless separately authorized. Initial site visits and stakeholder meetings are expected to be in person, with later meetings potentially held in person or virtually. The anticipated project period is approximately nine months.
