Description
Los Angeles County Department of Health Services is soliciting proposals from qualified entities to provide shuttle bus driver services at Harbor UCLA Medical Center and Olive View UCLA Medical Center. The contractor will operate shuttle buses transporting patients, visitors, and staff within the medical center campuses and provide reliable, safe transportation services. The solicitation requires submission of the County’s purchasing forms and supporting documentation. Appendix A identifies required forms including the Declaration, Proposer’s Organization Questionnaire/Affidavit, Proposer’s Certification of Compliance, Request for Preference Consideration, Commercially Useful Function Certification Form, Community Business Enterprise Information form, and Prevailing Wage County of Los Angeles form. Exhibit 6 must be submitted in Excel format. Applicable prevailing-wage, contractor-registration, certified-payroll, and subcontractor-compliance requirements also apply.
