ADVERTISEMENT SANTA FE COUNTY Clinical Guidance and Training Services for the CONNECT Network RFP No. 2027-0081-CSD/BM State of New Mexico Commodity Code- 91867; 91883 Santa Fe County and the Community Services Department (CSD) requests proposals from qualified licensed professionals or firms to provide Clinical Guidance and Training Services for CSD's navigators as well as interdisciplinary team development and meeting facilitation within the Santa Fe County CONNECT Network - in accordance with the Accountable Community of Health (AHC) standard of care. All proposals submitted shall be valid for ninety (90) days subject to action by the County. Santa Fe County reserves the right to reject any and all proposals in part or in whole. A completed proposal can be submitted in a sealed container, indicating the proposal title and number along with the Offeror's name and address clearly marked on the outside of the container or delivered electronically utiliz

Description
Santa Fe County's Community Services Department is seeking qualified licensed professionals or firms to provide clinical guidance and training for navigators within the CONNECT Network, interdisciplinary team development, and meeting facilitation in accordance with the Accountable Health Community standard of care. The scope includes trauma-informed care, client engagement, person-centered planning, professional boundaries, navigator education, professional development, mentoring, and facilitation of navigation network and interdisciplinary meetings, with a minimum of 50 hours of clinical guidance and meeting facilitation per quarter. The contract is scheduled to begin in November 2026, with an initial one-year term and options to extend for up to four years, contingent on funding and contractor performance. Proposals must address the specified evaluation factors, include relevant qualifications, experience, approach, scope-of-work response, past performance, three external references, professional licensure, an insurance certificate, a cost proposal, and a campaign contribution disclosure statement. The proposal is limited to 15 pages, excluding professional licenses and certifications, and must include a separate cost-proposal file when submitted electronically.
