Description
Los Angeles County's Department of Children and Family Services and Probation Department are seeking proposals from qualified organizations to provide Individualized Transition Skills Program services for transition-age youth ages 16 through 20. Services include individualized one-on-one life-skills training and coaching to help eligible foster and probation youth achieve self-sufficiency and the eight Chafee outcomes, including educational attainment, employment, housing stability, avoidance of high-risk behaviors, access to benefits and health resources, daily living skills, and permanent supportive relationships. The contract will serve youth in the County's North and South regions, with projected participation of up to 800 youth. Contractors must provide four to five billable service hours per enrolled youth each month, generally including at least 2.5 face-to-face hours, and may provide services for up to 24 months per youth. The scope also includes assessments, educational and vocational support, housing and community-resource linkage, group activities, participant incentives, case documentation, status updates, monthly and annual reporting, quality assurance, and coordination with County social workers, probation officers, and Independent Living Program staff. The maximum annual contract amount is $3,131,719, with one initial year and four additional one-year renewal options.
