Description
Lake County seeks a contractor to provide medical-grade oxygen and other compressed gases in certified cylinders for Fire Rescue, EMS, Fleet Management, and other County locations. The scope includes cylinder rentals, refills, exchanges, collection and replacement of cylinders, hydrostatic testing, monitor test gases, and related delivery and inventory services. Oxygen cylinders must be new aluminum cylinders, and the contractor must provide an online system for managing restocking, cylinder exchanges, and delivery scheduling, as well as online access to invoices, delivery tickets, testing reports, and safety data sheets. The contract is indefinite-quantity with no guaranteed volume or spending. The pricing sheet lists estimated quantities for evaluation purposes only. Bidders must submit the completed submission form, law compliance affidavit, revised pricing sheet, references form, proof of Sunbiz registration, W-9, SAM.gov registration, signed addenda, proof of insurance or evidence of insurability, and any required licenses. At least three verifiable references for similar projects completed within the past five years are required. The contract is for an initial one-year term, with two optional two-year renewals.
