Description
Orange County, New York, through its Department of Social Services, seeks a qualified, experienced agency to operate a health and medical services program for children in foster care. The scope includes accepting referrals from the Department, scheduling and performing initial comprehensive medical examinations within 30 days of placement, providing periodic examinations, immunization assessments, vision and hearing assessments, laboratory testing when appropriate, dental screening or referral, and observation and reporting of suspected abuse or maltreatment. The contractor must maintain medical files, obtain consents and provider documentation, complete the OCFS-0711 Child's Medical Record, prepare individual treatment plans, coordinate referrals and ongoing services, conduct weekly treatment-team meetings, educate children and caregivers, and submit quarterly and annual reports. The proposal must address services, staffing, consumer accessibility, monitoring and compliance, organizational qualifications and experience, references, pricing, and required certifications and forms. Staff must complete specified American Academy of Pediatrics trauma and resilience training within 30 days after award. The anticipated initial term is one year with up to four one-year renewal terms.
