Description
H-GAC is seeking qualified contractors to provide health maintenance services to older persons living in the region. Services include vision care (screening, prevention, treatment, eyeglasses), hearing care (screening, prevention, treatment, hearing aids), dental care (screening, prevention, treatment, dentures), prescription medications (excluding certain narcotics), and other health-related items such as equipment (shower chairs, transfer benches), supplies (diapers, gloves), and nutritional supplements. The contract will be awarded on a unit rate reimbursement basis with a 15% match for Title III B services and 25% for Title III E services. The solicitation remains open for four evaluation periods with deadlines on September 3, 2026, January 28, 2027, April 29, 2027, and July 29, 2027 at 12:00 PM CT.
