Gloucester County Improvement Authority seeks a New Jersey-licensed podiatrist to provide services for residents of Shady Lane Nursing Home from March 1, 2019, through February 28, 2020. Required services include monthly scheduled visits, ensuring each resident is seen at least quarterly, emergency visits as needed, compliance with applicable local, state, and federal regulations, and billing Medicare, Medicaid, and private-pay residents. The provider must coordinate wound care from the toes to the knee with the wound care practitioner, enter orders and resident documentation in the Sigma Care System, notify the facility's Director of Nursing about service changes, and label or engrave any dispensed braces or splints with the resident's name. The submission must include a current New Jersey podiatry license, current malpractice insurance, and a cost proposal covering any retainer, included services, and hourly billing rates. Travel expenses will not be reimbursed.
