Description
The Pennsylvania Department of Transportation seeks repair of an elevated, vertical 8,000-gallon asphalt emulsion tank. The work includes replacing the existing float indicator gauge with a compatible new gauge or approved equivalent, supplying and installing a 1/8-inch stainless-steel cable with pulleys and clamps, and removing the old cable. The contractor must provide the personnel and equipment needed to complete the repairs and submit modification-report documents for regulated storage tanks to the Pennsylvania Department of Environmental Protection. A qualified contractor must be a DEP-certified ACVL tank handler and must complete a site visit before the bid deadline to qualify. The bid package requires the Worker Protection and Investment Certification Form with the bid. It also includes the Reciprocal Limitations Act form, which must be completed and returned with the bid, and the Iran Free Procurement Certification Form for contracts of $1 million or more. The trade-secret notice form is available for use when applicable. Insurance coverage and a certificate of insurance are required before work begins or the contractor enters Department property; the insurance exhibit states that these are post-award obligations.
