Description
The Defense Logistics Agency is seeking one set of pseudoisochromatic vision-test plates, NSN 6540-00-299-8186, unit of issue set. The set consists of 16 plates, nominally 4.5 inches, including an optional plate for use with an astigmatic dial chart and specified vision test chart sets. The solicitation identifies Good-Lite Co. part numbers 133965-HRR and 730050 as applicable product references, but alternate offers require complete technical data describing the exact and alternate products. The item is an FDA-regulated Class I medical device. The supplier must provide commercial packaging capable of protecting the contents, package and mark the shipment in accordance with the stated medical marking and DLA packaging requirements, and deliver FOB destination. The quote must be submitted through the DLA Internet Bid Board System; the solicitation is not a small-business set-aside.
