Description
The City of Lebanon is soliciting best-value bids for the purchase and delivery of bulk propane to City facilities and refilling propane forklift cylinders. Bidders must provide a delivered firm price per gallon for bulk propane and a price per cylinder refill for forklift cylinders. Estimated usage is 10,000–15,000 gallons of propane and approximately 60 80-pound cylinders during the service period, although actual quantities may vary with weather and City demand. Deliveries must include all delivery, handling, surcharge, labor, fuel, insurance, and equipment costs, with at least 24 hours' advance notice and delivery during the City's stated receiving hours. The solicitation includes required bid pricing and agreement selections, a signed Appendix A Affidavit of Compliance, and a signed and notarized Work Authorization Affidavit with the federal E-Verify electronic signature page. Required insurance includes workers' compensation, commercial general liability, and business auto liability coverage at the specified limits.
