Description
This initiative by the Department of Health and Human Services aims to support innovative multi-level health care research for older adults, particularly those from populations experiencing health disparities. The research will focus on improving screening, diagnostic, and clinical care guidelines within primary care settings. It will also explore shared decision-making processes between clinicians, care teams, older adults, and their caregivers to enhance care planning and patient agency. Furthermore, the initiative seeks to identify effective strategies for care coordination. Eligibility is broad, including various governmental and non-governmental organizations such as county governments, independent school districts, city/township governments, public and state institutions of higher education, state governments, special district governments, federally recognized Native American tribal governments, other Native American tribal organizations, private institutions of higher education, nonprofits (with or without 501c3 status), small businesses, and for-profit organizations other than small businesses. However, non-domestic (non-U.S.) entities and foreign components are not eligible.
