Description
Licking County Job and Family Services is establishing a prequalification list of agencies interested in providing foster placement and/or residential treatment services for children. Inclusion on the list does not guarantee selection or a contract; agencies may be considered when placement services are needed. Providers must be licensed for the services offered, hold current State of Ohio certification, and carry adequate liability insurance. Applicants must submit a letter of interest describing the agency and its services, a copy of its foster care license, a copy of its State of Ohio certification, a certificate of liability insurance, and a rate list. To remain on the list, agencies must provide current copies of their active license, certification, insurance certificate, and rate sheet. The RFQ is open and ongoing through December 31, 2026.
