Public Health Solutions (PHS), selected as the Social Care Network (SCN) lead entity for Manhattan, Queens, and Brooklyn by the State of New York, is seeking applications from Health-Related Social Need (HRSN) service providers. This initiative, part of the New York State Department of Health's "New York Health Equity Reform: Social Care Networks" RFA #20417, aims to establish a comprehensive SCN. The network will screen, navigate, and deliver social care services to eligible Medicaid members to address their health-related social needs (HRSN). Participating providers will be reimbursed for eligible services rendered on behalf of the WholeYouNYC Social Care Network.
