Description
The Defense Logistics Agency is seeking nine adult CPR-D defibrillation electrode pad sets for use with Automated External Defibrillator Pro and Plus systems. The requested item is NSN 6515-01-519-6131, with the solicitation identifying Zoll Medical Corporation part number 8900-0800-01 and McKesson Medical-Surgical Government part number 510811 as approved or referenced sources. Alternate offers must include a complete data package for evaluation, including information for both the approved and alternate parts. The electrodes must be FDA-regulated medical devices and comply with applicable registration, listing, labeling, good manufacturing practice, packaging, marking, shelf-life, and domestic-material requirements. Each unit must have a 60-month non-extendable shelf life, with no more than nine months elapsed from manufacture to delivery. Delivery is required within 20 days after award, FOB destination, with destination inspection and acceptance. Packaging must comply with the stated commercial, ASTM D3951, DLA, and medical marking requirements.
