Description
The Utah Department of Health and Human Services is soliciting a podiatric physician to provide comprehensive podiatry services for Utah State Hospital patients. The scope includes consultations requested by DHHS physicians; diagnostic testing; treatment and preventive foot care; diabetic foot care; evaluation and treatment of foot pain or trauma; nail debridement and toenail removal; treatment of bunions, warts, fungal conditions, abscesses, calluses, and other foot conditions; orthotics; therapeutic injections; and related procedures. The contractor must manage and administer the podiatry services, document issues in the electronic chart, maintain records at the hospital, submit monthly reports, and provide services at times scheduled by the DHHS Clinical Director, estimated at approximately 12 hours per month without a guaranteed minimum. The contractor must generally provide the same podiatrist, notify the clinical staff of planned absences 60 days in advance, and comply with HIPAA through a Business Associate Agreement. Monthly invoices must be submitted with specified service, contractor, contract, and payment information.
