Description
Nebo School District is seeking a qualified, licensed Utah medical professional—an MD, DO, or APRN/NP—to serve as the District Medical Professional. The selected provider will work entirely remotely and provide district-wide medical authorization, standing orders and prescriptions, clinical consultation, protocol review, emergency medication guidance, support for individualized healthcare and emergency action plans, and clinical guidance to school nurses and health services staff. Applicants must demonstrate an unrestricted Utah professional license, active prescribing authority and applicable DEA registration, an active physical medical practice location in Utah, good professional standing, and professional liability insurance of at least $1,000,000 per occurrence and $3,000,000 aggregate. Responses must address professional experience, pediatric and school-health background, availability during school hours, urgent-response procedures, clinical expertise, references, and conflicts of interest. The submission also requires a current CV or resume, proof of the applicable Utah license, a certificate of insurance, and the completed Schedule A certification form. Fees are not submitted with the qualifications response; they will be negotiated after qualification-based ranking.
