Description
The New York State Office of Mental Health is soliciting bids for a statewide purchase authorization contract covering the supply and direct delivery of perforated specialty paper stock for meal tickets. The required product is white, smooth-finish, 24-pound, letter-size bond paper with three vertical, laser-compatible perforations. It is expected to be packaged and shipped by the case, with each case containing five reams of 500 tickets, or 2,500 tickets. Delivery must occur within a maximum of four weeks after receipt of an order. The contract will support as-needed orders from OMH psychiatric centers and facilities statewide. The bid quote sheet identifies an estimated quantity of 1,250 cases per year, with award based on the lowest total bid amount per item among responsible and responsive bidders meeting the mandatory requirements. Bidders must submit the specified forms, business references, experience documentation, insurance and licensing evidence, and applicable New York State compliance certifications.
