Submission Instructions
Description
The Utah Department of Health and Human Services is soliciting proposals from a single qualified provider to deliver physical therapy services at the Utah State Developmental Center under RFP DHHS26139. The contract is for five years and is not a guarantee that the selected provider will be requested to perform services. Offerors must demonstrate current Utah physical therapy licensure, and physical therapist assistant licensure if applicable; at least three years of professional experience serving individuals with developmental and cognitive disabilities and possible behavioral and physical issues; and current registration with the Utah Department of Commerce, Division of Corporations and Commercial Code. The cost proposal must be submitted separately from the technical proposal. Proposals must address all mandatory minimum requirements and be submitted electronically through Utah Public Procurement Place in PDF format. A completed cost proposal form is mandatory; exceptions to the RFP terms must be included with the proposal, and a Claim of Business Confidentiality form plus a redacted public-release copy are required only when confidential treatment is requested.
