Description
St. Clair County Community Mental Health is seeking residential service providers to deliver personal care and comprehensive community living support services in foster care and group homes. Services are authorized through individual plans of service and include care-plan implementation, staffing, emergency placement response, resident support, person-centered planning, documentation, claims submission, and coordination with clinical and compliance programs. The attached group-home placement protocol defines CMH approval and referral procedures, contractor responsibilities before and during placement meetings, required resident and health documentation, medication and personal-property processes, and post-placement reporting. Regular referrals are generally expected within seven days of approval, while emergency referrals may be made within 48 hours; visits should occur within five days of an approved referral, and sanctions of $250 per day may apply for delayed or denied placements without CMH agreement. The services require in-person staffing and compliance with applicable licensing, Medicaid, documentation, resident-rights, and quality requirements.
