Description
Myrtle Point School District 41 is requesting proposals for E-Rate-eligible Category 1 services consisting of Dedicated Internet Access and Wide Area Network transport. The FCC Form 470 identifies Internet access capacity of 1–5 Gbps for four entities and standalone data transmission service for three entities, with installation and initial configuration required. Providers must supply a valid Service Provider Identification Number (SPIN) and remain compliant with Schools and Libraries Division requirements throughout the contractual period. Proposals must comply with Oregon procurement laws and may not include excessive marketing or mass-email material. The district will not accept proposals by email; vendors must use the submission method specified in the associated RFP. The attached solicitation materials identify Attachment A, the Cost Proposal Form, as a required submission item. Pricing should address the requested Internet access and WAN transport services and applicable installation requirements.
