Description
The Washington State Department of Enterprise Services is soliciting bids to establish statewide contracts for medical, nutritional, and dietary food products and dietary supplements. The scope includes powdered and liquid formulas, vitamins, minerals, oral and enteral nutrition, infant formulas, disease-specific and metabolic medical foods, protein and fiber supplements, wound-healing nutrition, hydration products, and dysphagia-management products. Primary purchasers are Washington hospitals and correctional facilities, with use also available to other eligible public purchasers. WIC and Medicaid purchases are outside the scope. Bidders must complete and submit the Bidder's Certification and Price List. The Price List requires pricing for at least one item under each required category and includes required, non-required, additional-cost, and optional additional-item sections. A Subcontractor Inclusion Plan is required only when the bidder intends to use subcontractors. Bids must be emailed, with attachments in Microsoft Word, Microsoft Excel, or Adobe PDF format; zipped files are not accepted and the email, including attachments, may not exceed 30 MB. The resulting statewide contract is for up to 60 months, consisting of a 36-month initial term with a possible 24-month performance-based extension.
