Description
The Defense Logistics Agency is soliciting quotations for 154 packages of surgical eye shields under NSN 6515-01-598-1877: 150 packages for PR 7018427032, three packages for PR 7018422411, and one package for PR 7018431424. Each package contains 50 units and must include one aluminum Fox eye shield and one eye garter shield cover. The shields must be white and measure 60.3 mm by 76.2 mm. The items are FDA-regulated Class I devices and must meet applicable registration, listing, labeling, good manufacturing practice, packaging, and medical-marking requirements. Quotations must identify the proposed source and part number; alternate offers require a complete data package for evaluation. Delivery is required within five days after order, FOB destination, with destination inspection and acceptance. Packaging must comply with ASTM D3951, applicable DLA requirements, and Medical Marking Standard No. 1.
