Description
This opportunity is a request for an apprentice waiver to comply with Colorado Apprenticeship Requirements. The waiver is needed because no responsive, eligible subcontractors bid the project and there is no responsive, eligible Apprenticeship program within 50 miles one-way of the project. The form requires information on the Project Number, Subcontractor, Project Name, List of Trades, General Contractor, Phone, and Email. It also requires the submission of additional documentation or evidence to support the waiver request. The form is to be signed by the Printed Name, Title, and Signature of the requesting party. This form must be submitted to the Office of the State Architect prior to contract execution.
