Description
The City of Springfield, Oregon, is soliciting construction manager/general contractor services for City Hall deferred maintenance and quad redesign under RFP 4056. The scope concerns a specific civic-facility construction project. Attachment C, the Proposer Information and Certification Sheet, must be completed and signed by an authorized representative. It requires proposer identification and contact information, acknowledgment of the RFP and addenda, acceptance of a 180-day firm-offer period, tax and sanctions certifications, conflict-of-interest certification, and certification that the proposal was prepared truthfully and independently without collusion. The form also includes an Oregon Business Registry Number field if required.
