Gloucester County Improvement Authority is seeking a New Jersey-licensed psychiatrist or psychiatric nurse practitioner to provide routine and consulting psychiatric services for Shady Lane Nursing Home residents. Services include evaluations of new admissions with psychiatric diagnoses or medications; at least annual visits for residents with psychiatric, Alzheimer's, or dementia diagnoses; monthly scheduling; medication titration and documentation twice yearly; use of the Sigma Care system for orders and documentation; vacation coverage coordination; and notification to facility administration regarding service changes and coverage. The provider must offer grief counseling to residents and staff as needed, maintain all required qualifications, certifications, and licenses, and directly invoice Medicare, Medicaid, managed care organizations, private insurance, and other applicable payers. The cost proposal must include any proposed retainer, services covered by the retainer, and hourly billing rates; travel expenses will not be reimbursed.
