Description
Fairfax County's Community Policy and Management Team is accepting applications from current and prospective providers seeking Agreements to Purchase Services effective March 1, 2027, through June 30, 2030. The program serves eligible children, youth, and families in Fairfax County and the cities of Fairfax and Falls Church through a broad system of care. Applicants may propose private day school and special education-related services, congregate and residential care, psychiatric residential treatment, community-based services, treatment foster care, outpatient therapy and evaluations, and evidence-based treatments such as applied behavior analysis, dialectical behavior therapy, multisystemic therapy, parent-child interaction therapy, functional family therapy, and trauma-focused cognitive behavioral therapy. Providers must be appropriately licensed, submit organizational and clinician qualifications, service descriptions and rates, an organization chart, licenses and certifications, a W-9, vendor classification form, insurance certificate, and Medicaid documentation when applicable. The application also requires a completed provider application and clinician qualification forms, with resumes, licensure verification, certifications, and supporting service materials.
