Description
The provider will deliver drop-in Homemaker Personal Care and HPC Transportation services to support an adult woman with autism as she settles into her own apartment and maintains independence. Services may include transportation to medical appointments, shopping, errands, and community activities; appointment scheduling and follow-through; budgeting, grocery shopping, meal planning, and healthy-lifestyle support; medication and household reminders; assistance with daily routines; and orientation to community resources. The provider should build a positive, trusting relationship with the individual and offer calm, patient support related to anxiety, mood changes, withdrawal, and difficulty managing thoughts and emotions. Staff should encourage existing coping strategies, provide reassurance, and recognize when additional counseling or psychiatric support may be needed.
