Description
St. John's Community Health is seeking a qualified occupational healthcare provider to deliver comprehensive employee health services. The scope includes occupational health evaluations, workplace injury and illness management, workers' compensation coordination, employee physical examinations, pre-employment medical screenings, drug and alcohol testing, clinical testing, physical therapy and rehabilitation, return-to-work evaluations, Department of Transportation physicals, vaccinations, urgent care, telehealth, and virtual care. The provider will also support on-site employer health services, employee health and wellness programs, medical records and documentation management, employer reporting and communication, and workplace health and regulatory compliance. Proposals must include a cover letter signed by an authorized representative, a cost proposal worksheet, vendor experience, staffing information, examples of similar projects with at least three references, a project timeline and delivery estimate, and a list of vendors used. Any proposed outsourcing must be disclosed with the names and descriptions of the subcontracted organizations, and warranty information must be provided if applicable.
