The Gloucester County Improvement Authority seeks a New Jersey-licensed consulting physician to provide pre-employment and post-offer medical screenings, physical examinations, drug and alcohol testing, lift-task assessments, audiograms, N95 fit testing, TB screening, and other medical testing for employees and prospective employees. The scope includes annual medical examinations for CDL holders under New Jersey Department of Transportation guidelines and physicals required under union contracts. The provider will perform job analyses and position-specific screening recommendations, functional capacity, fit-for-duty, and return-to-work evaluations. Appointments must be available within five working days of request, results must be summarized and forwarded to the employer, and medical personnel must work directly with Human Resources. Billing must be submitted on an ICD-9 form to the designated third-party administrator. The proposal must include pricing for pre-employment physicals and testing, non-DOT and DOT physicals, drug screens, TB screening, chest X-rays, laboratory work, Titmus eye screening, hearing tests, pulmonary function tests, and EKGs.
