Description
The Ohio Department of Behavioral Health, through Ohio Pharmacy Services, is seeking a supplier for Theranova US 400 dialyzers used in the treatment of chronic kidney disease. The anticipated usage is approximately 500 cases annually, purchased monthly, although quantities may change and the agency reserves the right to purchase up to the full specified quantity. The award will be made to the lowest-cost responsive and responsible supplier. Pricing must be entered in the OhioBuys item grid, and any separate quote must exactly match the item-grid total. Delivery, setup, warranty, and all other applicable costs must be included. Substitutions are generally not permitted unless authorized in the solicitation; any authorized substitute must closely resemble the specified product and is subject to facility review. Vendor inquiries must be submitted exclusively through the OhioBuys Inquiry Field.
